Incident Narrative

An employee was using an A-frame ladder to change a lightbulb in a barn. The lightbulb slipped and fell to the ground. As the employee went down the ladder to retrieve the lightbulb, his foot slipped and he fell approximately 3 feet to the ground. The employee sustained three left hip fractures and abrasions to the elbow.
