Incident Narrative
An employee was standing a coil from a skid and placing it on an un-coiler using a hoist rigged with a mesh sling. The coil shifted and the employee attempted to stop the coil from shifting with his left hand while his right hand was on the hoist pendant controls to raise the coil to the vertical position. The employee's left middle finger was then pinched between the coil and the mesh sling, resulting in amputation of the finger pad.
