Incident Narrative

An employee was putting up a tent for an outdoor event. As the employee was inserting the steel bar into the vinyl pocket, a gust of wind caused the panel to flap and the steel bar to come loose. The bar then impacted and caught the employee's right index finger between it and the aluminum frame of the tent. His right index finger was partially amputated.
