Incident Narrative
An employee was replacing bolts on the sawmill chip bin auger coupling. As he was replacing the last bolt, the system activated, catching his leg and causing him to fall through the auger system. He sustained a below-the-knee amputation to his left leg. The employee was wearing fall protection and was caught by his lanyard during the fall.
