Incident Narrative

Two employees were in a man basket removing forms from under a bridge. After the employees removed a C-channel from the final form, the C-channel was supported by the man basket handrail to be lowered to the ground. The employee's hand was resting on the handrail on the descent when the C-channel rolled, crushing his hand between it and the handrail, resulting in an amputation.
