Incident Narrative

An employee was working on a belt in an air handling unit and disconnected the power source. As the fan belt was coming to a complete stop, the employee's left fourth fingertip was amputated. The employee also sustained a displaced fracture of the left second finger and a laceration to the third finger at the distal-most joint.
