Incident Narrative
An employee had drained a sprinkler system through a hose to the parking lot. The employee lifted the metal grate covering the parking lot drain to get the hose more securely into the drain. When lowering the grate back into place, the grate snagged and dropped, catching the employee s right middle finger and amputating it to the first knuckle.
