Incident Narrative
An employee was prepping pulp material to load into a pulp machine. The 420-pound pulp bale was elevated by a forklift and the employee was standing in front of the lift to cut the wire banding from the bale in order for the pulp to be loaded into the pulp machine. When the last banding was cut, the tension was released and the bales fell. A bale landed on the employee's right leg, resulting in a hip or femur fracture.
