Incident Narrative
An employee was on a ladder working on a duct fan motor in a parking garage. After testing the fan and motor, the employee climbed up the ladder to retrieve his tools. He contacted the fan belt of the drive motor while trying to steady himself on the ladder. The fan belt rotated and trapped his finger between the drive motor pulley and the belt, resulting in a middle finger amputation.
