Incident Narrative

On March 16, 2023, an employee was utilizing a forklift outside of the facility to clean up trash when the vehicle became stuck. The employee then went into the building to use another forklift to pull the stuck forklift out with a chain/hook. While pulling out the stuck forklift, the chain and hook broke, striking the employee in the head/face. The employee sustained injuries to the head that included a fractured skull, brain bleed, the loss of seven teeth, and lacerations. The employee was hospitalized.
