Incident Narrative

An employee was feeding material through the rollers of a machine when his hand became stuck in the machine. The employee removed his hand and continued to work. The employee then noticed his hand was bleeding and became lightheaded; the employee passed out and fell, striking his head on the floor. He sustained a skull fracture.
