Incident Narrative

Employees were trying to set a 25-foot ladder beside a furnace. They could not get it into place on their first attempt, so they moved the telescoping forklift they were using to another position. During the second attempt, the boom/forks caught on a valve/pipe on a steam-containing cooling line. The motion of the lift broke the line; steam was released, causing second-degree burns to one employee's right forearm, right thigh, stomach, and abdomen.
