An employee was grabbing his personal belongings from a supply shelf in a backroom. The shelf was above a pallet of butcher wrap paper that had a zip tie on it. The employee's foot became caught in the zip tie, causing him to lose his balance and fall onto the floor on his left side. The employee sustained a hip fracture and was hospitalized.
