Incident Narrative

An employee had completed packing materials onto a moveable pack stand. While pushing the pack stand, a wheel became stuck, causing the pack stand to tip over. Bundled metal material weighing 1,051 pounds fell 33 inches from the pack stand onto the employee's right foot. The employee sustained an amputation.
