Incident Narrative
An employee was rigging equipment to be placed on the tank roof. While setting up containment rigging, the steel outriggers slipped from the employee's hand and slammed on top of his finger, crushing it against the exterior tank roof. The employee sustained a crush injury to the left index fingertip and nail bed damage that resulted in an amputation. Note to coder- amputation
