Incident Narrative

An employee was jogging bags on a form fill and seal machine when a bag became stuck. The employee opened the jaws of the unit to remove the bags. When placing the bags back on the jaws, the jog button was pressed, causing the machine to clamp onto the employee's finger. They sustained a left index fingertip amputation.
