Incident Narrative
An employee was loading a forklift with a 7,500-pound steel rod using an overhead crane. As the employee set the rod on the forks, the rod began to roll forward. The employee placed his foot at the end of the forks to stop it from rolling off when the steel rod rolled onto the employee's foot, resulting in five right toe fractures.
