Incident Narrative

On February 8, 2023, a team in the molding department was processing a 240-ton axle mold. The cope of the mold was rigged with lifting chains, hoisted off of the pattern, and set on two wooden blocks. As the employee unhooked the chains, the flask on the blocks rocked, causing the employee's foot to become pinched. Three toes were amputated.
