Incident Narrative

Employees had just finished running a metal culvert pipe. After making the culvert, the pipe is cut loose from the corrugation mill and is then cut manually by pipe wrenches. When the cut was completed, the employees were heading back to the mill area and the table started to dump. The culvert pipe fell on the employee, resulting in a broken left arm.
