Incident Narrative

An employee was spotting a forklift as it moved structural steel beams at a construction site. The forklift set two of the beams down on cribbing. The employee was separating the beams when one of them shifted; the employee's right middle finger was caught between the two flanges on the beam, and the fingertip was amputated.
