At about 3:00 p.m. on February 3, 2023, an employee was spraying foam insulation in a bonus room when he fell through a 3-foot by 6-foot floor hole. He landed on the concrete garage floor 11 feet below and sustained a rib fracture, a shoulder injury, a punctured lung, and a scalp contusion.
