Incident Narrative
A piece of steel plate was being hoisted by a crane into a building. The chain used to lift it came loose from the lifting hook, due to ice on the plate, and the plate fell. It landed on an employee's legs and feet. He was hospitalized for lacerations on both shins that required stitches, as well as an injury to the right foot that required surgery.
