Incident Narrative

An employee was operating a forklift to load pallets inside of trailers. While on the forklift inside of a trailer, the employee reached through the forks from her seat to place a shipping label on a pallet. The controls were pressed and the pallets came down on her right forearm, resulting in a fracture in two places.
