Incident Narrative

An employee reported to work at 2:00 p.m. and conducted a pre-shift inspection on a stand-up powered industrial vehicle (PIV). The employee was standing on the PIV with their hands over their head when they fell 10 inches from the forklift platform onto the concrete floor, resulting in injuries to their back and to the back of their head.
