Incident Narrative
A crew was using a forklift to lower the frame of a tent structure. A bracing strap became caught in the mast of the forklift, causing the frame to twist and create tension. The employee went to remove the eye bolt at the apex when the tension on the forks released. The frame sprung back and struck the employee in the face and head, resulting in a laceration and contusion.
