Incident Narrative
An employee was operating a forklift to travel between buildings. As the lift was moving down the ramp, they reached out to press the open button for the overhead door. While pulling their arm back into the lift, the lift started to move down the ramp, catching their arm between a bollard and the forklift frame resulting in a left radius and ulna fracture.
