Incident Narrative
On December 22, 2022, at approximately 8:50 AM, two employees were cleaning an above ground, horizontally placed, 45,000-gallon gasoline tank. The two employees were standing on top of the tank s working platform when the tank exploded. Both employees were blown away from the platform and landed on the ground. Employee 1 was treated for a broken ankle and released. Employee 2 was hospitalized for a head injury.
