Incident Narrative

An employee was standing on an A-frame ladder, adjusting a rail on an overhead shop door with a pry bar. The pry bar slipped from the rail, the ladder became unstable, and the employee jumped to the concrete floor below. He landed on his feet, resulting in a compound fracture to his left foot.
