Incident Narrative

A maintenance employee had just replaced a non-idler shaft roller on a conveyor and noticed an issue with the tension roller. While putting pressure on the belt and roller to make adjustments, the energized conveyor belt caught his hand between the roller and the belt. The employee sustained a forearm fracture and laceration.
