Incident Narrative

An employee was operating a shavings mill machine that cuts tree logs into wood chips. The machine's sensor eye slipped from position and the employee stepped onto the cart full of logs to fix the sensor. The machine turned on, shifting the log cart and the employee slipped forward and grabbed the cables to catch himself. The cables pinched the employee's fingers and hand against the rotating pulley, resulting in multiple finger amputations on both hands.
