Incident Narrative

An employee had been cutting plate with an acetylene torch. The employee left for lunch, during which the regulator was turned off. When the employee returned from lunch, he turned the valve to begin work and a fire erupted from the oxygen tank regulator. The employee sustained burns to the stomach area.
