At approximately 8:40 AM on November 21, 2022, an associate was building a 51-inch bladder and removing spacer bolts by raising the lock ring top with a prybar. The prybar slipped out from its position, causing the lock ring top to land on the tip of the associate s left index finger, resulting in a fingertip amputation.
