Incident Narrative

An employee was conducting a security walkthrough of a warehouse. The power went out. As the employee was walking back through the building toward the security post, a forward moving forklift struck the employee; its load then slid off its forks and fell onto the employee. The employee suffered a broken pelvis and ribs and was hospitalized.
