Incident Narrative
An employee was preparing to weld a steel beam that was on work horses in front of him. The employee used a wireless control to retract the sling of a crane back to the safe position. The sling snagged an angle iron that was attached to the beam, causing the beam to topple over and push the employee backward into a stack of steel. He then fell to the floor and the beam landed on his lower right leg, resulting in fractures to the tibia and fibula, and a laceration to the back of the head.
