Incident Narrative

On October 31, 2022, an employee was walking across a 3:12 pitched metal roof of a warehouse to check the gutters. The employee stepped onto a skylight, causing him to fall approximately 20 feet onto metal tubing and the concrete floor below. The employee was hospitalized after sustaining fractures to the ribs, wrists, and vertebrae as well as neck injuries and a head laceration.
