On October 31, 2022, at approximately 2:13 PM, two employees were inspecting a crankshaft on rollers. Employee 1 was rotating the crankshaft manually as employee 2 was checking a gauge. Employee 2's hand was caught in a pinch point between the rollers and the counterweight of the crankshaft, resulting in an a partial amputation of the right thumb.
