Incident Narrative

An employee was climbing down a structure. He climbed down a ladder, then fell through an adjacent open grate that was 10 feet above a lower level. He landed on that level with the lower half of his body dangling over another open grate, began to fall through that grate as well, but then was able to stop his fall. He was hospitalized for surgery, having broken his leg in the first fall.
