Incident Narrative

An employee was using a punch press to punch a magnetic strip when the material jammed. When the employee attempted to unjam the press, their left index, middle and ring fingers were caught in the press. The employee sustained an amputation to the tip of their left middle finger and received stitches to their left middle, ring, and index fingers.
