Incident Narrative
Excess molten iron was moved to a pig drain pond. When some of the iron had cooled, a forklift was used to break up and move iron that had overflowed from the pond. Some molten iron melted a hydraulic line on the forklift, causing pressurized hydraulic fluid to spray out. The fluid was then ignited by the molten iron, resulting in a fireball that burned the hands and face of an employee who was standing on the melt deck about 14 feet away from the forklift. The injured employee was hospitalized.
