Incident Narrative

An employee was operating a forklift on the mezzanine of a parking area to relocate pallets of materials. The top of the forklift struck an overhead beam, causing the employee's head to hit the steering wheel. The forklift then tipped over and pinned the employee's right leg underneath it. The employee suffered a laceration to the head and a right leg fracture.
