Incident Narrative

A crew was tarping haystacks using a telehandler. An employee got onto the forks of the telehandler to be lifted up to the top of the haystack with the tarps. The employee attempted to hook the loops on the tarp to the hooks on the telehandler. The telehandler moved and caught the employee's arm, breaking and lacerating it. The employee was hospitalized, requiring surgery.
