Incident Narrative
An employee was performing maintenance on a conveyor system that was not operational. There was another operational conveyor system nearby; the employee removed the safety covering from the operational pusher to help determine why the other pusher was not operating. The test switch was pressed and the pusher went over the employee's right thumb, resulting in a partial amputation. The thumb was later fully amputated.
