Incident Narrative

Two employees were adjusting a 2-foot by 3-foot, 150-pound catch basin. They had removed the casting, which was sitting half on the road and half on a frame. The casting dropped and crushed the tip of an employee's finger against a brick being used to hold the casting in place. The fingertip was amputated.
