Employees were making a tie-in on an 8-inch natural gas line by attaching two squeeze-off tools to shut off the line. When the polyethylene pipe was cut, one of the squeeze-off tools failed, causing a leak that resulted in an ignition. One employee was burned over about 50 percent of their body; the other was burned over about 10 percent.
