An employee was inspecting a roll of fabric in an accumulator. The fabric was moving to one side inside the accumulator, so the employee stopped the machine and moved the fabric back to the correct position. The employee restarted the machine and while exiting the machine became caught in it, resulting in a left arm amputation.
