Incident Narrative
On October 3, 2022, at around 5:00 PM, an employee-in-training was changing out a commercial paper roll on a corrugator inside the facility warehouse. Once the roll was installed on the roll stand and ready to be put into production, the employee went to clear the excess paper from the roll. His right index finger was caught between the clamp and prep bar, resulting in an amputation of the tip that was later reattached.
