Incident Narrative
An employee was cutting off the tops of screws protruding through plywood, using an angle grinder with an abrasive cutoff wheel. The wheel exploded, and shrapnel from it severely lacerated the employee's left forearm and damaged an artery. The grinder was unguarded at the time.
