Incident Narrative
An employee had just completed a lock out/tag out of a machine and was activating the equipment to check and track the conveyor belt. The employee's hand was struck by the belt and his right index, middle, and ring fingers were caught in the unguarded belt and pulley. The employee's right index finger was amputated at the first knuckle, and he sustained fractures, contusions, and lacerations to his right middle and ring fingers.
