Incident Narrative

An employee was reassembling the roller frame of a bulldozer. The roller frame was suspended from an overhead crane and bolted to the frame of the bulldozer. The employee was aligning the equalizer pin to reattach the roller frame, which was hung up on the equalizer bar. He was under the suspended roller frame, trying to use a sledgehammer to drive the equalizer pin out; when the pin was struck, the frame shifted off the equalizer bar downward about 4-6 inches and struck his left hip area. He suffered a fracture to the pelvic region.
