Incident Narrative
At about 3:20 p.m. on September 14, 2022, an employee was reaching overhead to retrieve an empty 30-gallon drum from a storage rack. Upon retrieving the drum, he lost his balance and fell to the floor. He struck his head, and the drum struck him across the leg; he suffered a laceration to the leg as well as head trauma.
